Request an extension of care
Request for extended care can be submitted by completing the brief request form HERE
All requests for extension to cover over a weekend will automatically be approved for funding. One of our team will review ongoing requirements and contact you next business day.
If the services need to be put in place outside of normal hours, you must contact the care provider directly (refer above for contact details).
Urgent Residential Care Placement
Requiring urgent care placement out of hours
POAC does not coordinate placements in aged care facilities outside of normal business hours.
If patient requires urgent placement and a care facility has accepted the patient directly as an after-hours admission, POAC will fund the initial period of care and contact the referrer on the next business day to discuss further requirements. The arrangements must be made directly between the care facility management and the referrer and/or GP.

Radiology Services
Ultrasound or CT request
POAC coordination is required for all CT and USS requests. Bookings are able to be made only during normal business hours when radiology booking teams are available.
Refer and follow guidance provided by the Auckland Regional HealthPathway, or contact the relevant hospital service for advice. For urgent clinical queries that would potentially change immediate management, Sarah Hyder, POAC Nurse Advisor may be contacted on 021681319.
Send us a radiology request form HERE and POAC will contact your clinic on the next business day with appointment details. Or prepare a radiology request from in your normal way and email it to us referral@poac.co.nz. Providing a contact mobile number for the referrer is helpful.
X-Ray
Refer acutely required requests to your local private radiology provider. Normal POAC funding rules apply.

TRANSPORTATION
POAC Urgent Care Clinics and St John ambulance crew/call centre staff have access and authorisation to book travel to/from locations related to a POAC patient's medical care (where normal POAC entry criteria met).
If this information is not available after-hours by the medical care provider, POAC will fully reimburse any related travel for a valid POAC case where the patient or provider has incurred costs for this travel.
