Home-Based Support Services (short-term only)

Short-term assistance with personal care, medication prompting, meal preparation, palliative support, and other essential home-based services.

This is a short-term service providing care through acute illness, emergency social situations, or bridging a gap between funding streams or alternative services being available.  Call to discuss if extended care (>2 weeks) is required.

Ensure long-term requirements are considered and refer to NASC where required.  

Exclusions:

• ACC-related care (refer directly to ACC)

• POAC does NOT fund shopping or house cleaning activities.  

• Post-operative support requests should be sent to local NASC service
