POAC Acute and Planned Care Services
POAC provides a range of acute and planned proactive services for the Auckland Region.
Referrals can be made directly via the PMS, via one of our online forms, or contact our team to discuss by phone (09) 535 7218 (Mon-Fri) or email referral@poac.co.nz
The POAC service across the Auckland Region facilitates access to existing infrastructure and resources in the community to provide a range of funded services that prevents an acute hospital attendance, or shortens the length of hospital stay for patients who do attend or are admitted to a public hospital in the Auckland, Counties Manukau or Waitemata District,
The POAC Care Co-ordination Service is available Mon-Fri to schedule acute and urgent appointments for USS, CT, arrange in home support services, support short-term placement in aged care facilities, transportation, infusions and more.
POAC clinical policies have been developed to support safe clinical practice and are aligned with the Auckland Regional HealthPathways.

POAC funds and coordinates access to community-based radiology investigations as an alternative to acute referral to hospital.
ACC and Maternity funding should always be used in the first instance and are generally excluded from POAC funding.

WOMENS HEALTH
Funded community access to women's health services are available for eligible patients across the region, including:
ST JOHN
The St John and POAC initiative across Auckland Metro provides a clinically safe alternative to transferring a patient to the hospital emergency department. Patients can be seen and treated at an Urgent Care Clinic or General Practice following in-person assessment, or telephone triage via the St John Clinical Hub.
